I’m Dr Adam Pisani, practice partner at Wellbeing Chiropractic and the AHPRA registered chiropractor leading our Wyndham Vale clinic, with weekly consulting at Caroline Springs. A double degree in health science and chiropractic pulled me early toward the hard end of the caseload, the long-standing back pain, sciatica, headaches and vertigo that have usually outlasted a few fixes already. When it’s been years, not weeks

Back pain you’ve stopped counting in weeks

Low back pain that started years ago and never really left, now with sciatica burning down one leg on the Princes Freeway drive home. Somewhere along the way you stopped calling it an injury.

A headache that keeps a schedule

It starts at the base of the skull midweek and builds. By Friday you’re dimming screens and cancelling plans, hoping it stays a headache and doesn’t tip into a migraine.

Rolling over in bed sets the room spinning, and so does looking up to a high shelf. You’ve started moving through your own house carefully, and planning around it.

Most of what I see at Wyndham Vale has history behind it. Chronic low back pain, sciatica, headaches and migraines, vertigo, rib and shoulder pain, posture that’s been complaining for years. Pain with that much history has usually changed how you move, sleep and plan, which means a quick once-over misses most of the picture. So the first appointment digs: what started it, what keeps setting it off, what you’ve already tried. If what I find isn’t a chiropractic problem, I’ll tell you that too, and refer you on.

The caseload I went looking for

I hold a double degree, a Bachelor of Health Science alongside a Bachelor of Applied Science (Chiropractic), and gravitated to chronic pain almost immediately. It’s demanding work. The person in front of you has usually tried a few things already, and they deserve better than a repeat of whatever didn’t work last time.

These days I’m a partner at Wellbeing Chiropractic and lead our Wyndham Vale clinic, with a caseload that runs from the very young through to the elderly. That range is deliberate. Working across six techniques, from Gonstead through to Activator and pelvic blocking, means I can adapt the care to the person on the table rather than asking them to adapt to me. In 2024 I was named Chiropractor of the Year, which I’ll state once, plainly, and get back to work.

Caroline Springs is next, with a new clinic opening soon on Caroline Springs Boulevard. The reason I keep at it hasn’t changed since my first year in practice: the difference the right care can make to someone’s health and wellbeing, and the plans they start making again once pain stops being the loudest thing in their week.

Assessment first, technique second

Your first visit is mostly questions and testing, and I make no apology for that. I want the full history, not the last fortnight: how it started, what it interrupts, what you’ve already tried. Then a thorough physical assessment of how you actually move, because with long-standing pain the place that hurts and the thing that’s driving it are often different.

Only then do we talk technique. I work across Diversified, Gonstead, Activator, drop piece, pelvic blocking and soft tissue and rehab work, and the point of that range is choice. A low-force instrument adjustment might suit an older patient or a young child. A specific manual adjustment might be the better call for someone else. Your care plan gets built around your presentation, then reviewed as you go, not set once and left running.

How I work in the treatment room

Diversified

The hands-on adjusting most people picture. Specific, manual work aimed at restoring movement to joints that have stopped moving well.

Gonstead

Analysis before adjustment. A detailed system for finding exactly which segment is involved and adjusting it specifically, nothing broad-brush.

Activator

A spring-loaded instrument with a light, targeted click of an impulse. It suits many of my youngest and oldest patients.

Drop piece & pelvic blocking

Table-assisted methods that use positioning and a controlled drop to lower the force required. Often my choice for pelvic and low back presentations.

Rehab & soft tissue therapy

Muscle and soft tissue work plus targeted exercise, so what we do on the table has support between appointments.

Questions patients ask

Yes, and the assessment is the point. Pain that’s been around for years has usually changed how you move and compensate, so I spend the first visit working out what’s driving it now, which isn’t always what started it. Then I’ll give you a clear read on whether chiropractic care fits your presentation, and what a realistic care plan looks like. No promises of quick fixes. You get a workable plan with regular reviews.

Because working out which type of vertigo you have comes first, and that’s an assessment job. Some presentations involve the neck or respond to specific positional techniques, and those are worth assessing properly. Others belong with your GP, and if that’s what your assessment points to, I’ll refer you rather than treat you.

Yes, we have HICAPS, so if your extras cover includes chiropractic you can claim on the spot with all major funds and only pay the gap. Your level of cover is set by your fund, so check your policy if you’re unsure what it includes.

No referral is needed to book. Bring anything useful you already have: scans, imaging reports, a medication list. Wear something you can move in. First visits run longer than a standard appointment because the history matters, especially with long-standing pain.
